CareScope's take

The only useful question about backup is how long it takes to see patients again, and Datto is built to answer it: local appliance for speed, cloud copy for the fire, and the ability to run systems while the original is broken. Because it is almost always sold through a managed service provider, the quality of your experience is really the quality of that provider. Insist on a documented restore test you witness, with a stated recovery time. A backup nobody has restored is a hope, not a plan.
